Youth Leader Packet
The Youth Leader must complete the Group Leader Agreement in this packet and mail it in with your payment of your remaining balance. This Agreement and balance are due twelve days prior to the start of camp.
The Youth Leader must also complete the Group Rooming Information Form and email it to rebekah@radiuscamps.com by May 15, 2009. This form provides us the information we need to prepare adequate spacing arrangements for the breakdown of your group and the information we need for your camp shirts.
Student/Adult Participant Packet
Each participant (students and leaders) needs to have a completed packet. Note that forms for students under the age of 19 must be completed by legal guardians. These forms are to be brought to camp on the day of arrival to be turned in at registration. Included in this packet you will find important information of what to bring and not to bring to camp.
Each participating church is required to have medical information for their students. This Sample Medical Release and Treatment Form is a guide for you to make your own form. Note that it may not meet the requirements for all states.
